Advance care planning gives you the opportunity to make those decisions now, while you&#8217;re healthy and clear-headed, so your family never has to guess what you would have wanted.<\/p>\r\n<h2>What Is Advance Care Planning?<\/h2>\r\n<figure><img loading=\"lazy\" decoding=\"async\" title=\"What-Is-Advance-Care-Planning-1920w\" src=\"https:\/\/qfbwellness.com\/healingatthecenter\/wp-content\/uploads\/sites\/2\/2026\/09\/What-Is-Advance-Care-Planning-1920w.jpg\" alt=\"Person holding another person's hand for comfort\" width=\"1280\" height=\"522\" \/><\/figure>\r\n<p>Advance care planning is the opportunity to make two critical decisions while you&#8217;re still able to: first, identifying someone who can speak for you when you can&#8217;t speak for yourself, and second, telling the people you love and trust about the quality of care you want at the end of your life.<\/p>\r\n<p>Like other Advance Care Planning (ACP) specialists, I am trained to guide you through completing your Advance Directive documents and navigating the important decisions they require.<\/p>\r\n<p>What sets my approach apart is my background as a Certified Grief Coach and Educator. I understand that advance care planning is not only a practical process-it is also an emotional one. Grief can surface in many ways, including anticipatory grief when considering future loss, stress related to family dynamics or disagreements, and the quieter, often unspoken sense of loss that can arise when facing mortality.<\/p>\r\n<p>In addition to ensuring your documents are completed accurately and reflect your wishes, I have the skills and training to support you in acknowledging and working through these emotional experiences. This creates space for greater clarity, confidence, and peace of mind-for both you and those who care about you.<\/p>\r\n<h2>Individual Facilitation<\/h2>\r\n<p>I work one-on-one with individuals and families to guide the advance care planning process from start to finish. This includes identifying a healthcare representative, facilitating conversations about care preferences, and ensuring that your wishes are documented clearly and accessible to the people who need them.<\/p>\r\n<p>These sessions are compassionate, unhurried, and designed to meet you exactly where you are &#8211; whether you&#8217;re starting from scratch or updating an existing plan.<\/p>\r\n<h2>Community and Group Workshops<\/h2>\r\n<p>Advance care planning workshops are offered regularly through Healing at the Center \u2122&#8217;s community education programming. These group workshops walk participants through the basics of advance care planning in a supportive, accessible setting.
